Registration

Forms and payment are due by (deadline) January 20, 2012.

You can submit your Entry Form and Player Roster Form online or you may print the forms from this website and mail or fax them in before the deadline.

Payments can only be mailed.  You can make your payment with a Cashier's Check, money order  (No Personal Checks!)  or by charging your payment using your Visa, MasterCard or American Express.  Registration fee (if received by January 6) is $425.  After January 6, the registration fee is $450.

The registration fee is $425 U.S. funds if payment is received by

Friday, January 6.  After that date, the registration fee is
